Clarithromycin and itraconazole are substrates and inhibitors of CYP3A, in connection with which clarithromycin may increase the plasma levels of itraconazole and vice versa. When used with clarithromycin itraconazole patients should be closely monitored for the detection of amplified or symptoms or prolonged pharmacological effect.
Cost of biaxin
The use of clarithromycin (500 mg two times daily) with saquinavir (soft gelatin capsules, 1200 mg 3 times daily), which are CYP3A substrates and inhibitors, in 12 healthy volunteers resulted in an increase in AUC equilibrium by 177% and Cmax for 187% compared with the use of only saquinavir. Thus AUC and Cmax of clarithromycin were increased by approximately 40% with the use only of clarithromycin. No need to adjust doses if both drugs are used simultaneously for a limited period of time and in the above doses and dosage forms. Results of the study drug interactions with the use of soft gelatin capsules may not reflect the effects of that seen with saquinavir in the form of hard gelatin capsules. Results of the study drug interactions with the use of saquinavir alone may not fully reflect the effects observed during treatment with saquinavir / ritonavir. When used in conjunction with saquinavir ritonavir must take into account the possible effects of ritonavir on clarithromycin (see. Above).

Pharmacodynamics.
Clarithromycin - a semisynthetic antibiotic macrolide. Antibacterial action of clarithromycin is determined by its binding to ribosomal subunit 5OS-sensitive bacteria and inhibition of protein synthesis. The drug shows high efficacy in vitro against a broad spectrum of aerobic and anaerobic gram-positive and gram-negative microorganisms, including hospital strains. The minimum inhibitory concentration (MIC) of clarithromycin are usually twice lower than the MIC of erythromycin.
